Understanding your Insurance Plan

We participate with most local and national insurance plans. Because each health insurance plan is different, we recommend that you contact your insurance company to better understand your unique benefits and plan requirements. If we are not participating with your health plan, we will ask for payment at time of service. Our fees vary depending on the complexity of your diagnosis and the services provided. Please present your insurance cards, identification card and referral numbers to the receptionist upon your arrival. Payment is expected at the time of service for all patient balances

It is important to note that our physicians perform several procedures in the office. Please be aware that often these procedures are not inclusive in your office copay; out of pocket expenses such as deductible and coinsurance may apply. If you have any questions about your coverage, please contact your insurance carrier.

The insurance plans listed below are generally accepted by our practice. To make sure we accept your specific insurance plan, please contact your insurance carrier prior to your appointment. We do not participate or accept any plans from the Affordable Care Act Health Exchanges.
